Job summary

21st Century Home Health Services (21HHS) is seeking a Regional Quality Manager to elevate patient care across Northern California. You will own clinical quality across the region, build scalable processes, and drive performance improvement across multiple sites in Walnut Creek, San Jose, Burlingame, and surrounding offices.

This onsite leadership role demands California licensure, a Bachelor’s in a related field, 2+ years in home health or healthcare operations, and a proven ability to coach

Qualifications

• California Clinical License required for practice in CA (RN, PT, ST, OT).
• Bachelor’s degree in healthcare administration, business, nursing, or related is a plus.
• 2+ years in home health or healthcare operations is highly preferred.
• Multi-site or regional support experience is a plus.
• Outstanding customer service skills are essential.

Responsibilities

• Own the regional quality function — set priorities and standards across sites.
• Monitor patient outcomes, readmission rates, infection rates, and satisfaction; drive improvements.
• Lead performance improvement projects from problem identification to results measurement.
• Standardize best practices and ensure clinicians can follow processes in the field.
• Coach clinical teams with real-time feedback and targeted education.
• Prepare region for surveys and accreditation reviews; maintain audit readiness.
• Build and maintain quality infrastructure (dashboards, audits, reporting cadences).
• Partner with leadership to align quality goals with staffing, scheduling, and growth.
