Job summary

21st Century Home Health Services (21HHS) seeks a Regional Quality Manager to lead clinical quality across multiple California sites, including Walnut Creek, San Jose, Burlingame and Sacramento. You will build systems, dashboards, and training to drive patient outcomes, reduce readmissions, and improve accreditation readiness.

This role requires hands-on leadership and a passion for coaching teams to higher standards.

Qualifications

• California Clinical License (Nursing, PT, ST, OT) preferred.
• 2+ years in home health or healthcare operations.
• Multi-site or regional support experience is a plus.

Responsibilities

• Own clinical quality across the region and translate data into practice changes.
• Track patient outcomes, hospitalizations, readmissions, infection rates, and patient satisfaction; drive action.
• Lead performance improvement projects from identification to measurement of results.
• Standardize best practices across branches for consistent field adoption.
• Coach and develop clinical teams with real-time feedback and education.
• Investigate incidents and adverse events with a systems view to prevent recurrence.
• Partner with regional and operational leadership to align quality goals with staffing and growth.
• Prepare the region for surveys and accreditation reviews with ongoing readiness.
• Build quality infrastructure (dashboards, audit tools, reporting cadences) for visibility.
• Foster accountability and psychological safety so quality is everyone's job.
