Job summary

21st Century Home Health Services in Northern California is seeking a Regional Quality Manager to set the standard for care delivery across multiple sites. This onsite role will oversee clinical quality, performance improvement, and a data-driven infrastructure in Walnut Creek, San Jose, Burlingame, Sacramento.

You will build dashboards, coordinate audits, coach clinicians, and partner with regional leadership to align quality with staffing and growth plans.

Qualifications

• California Clinical License (RN, PT, ST, OT) required or strongly preferred.
• 2+ years in home health or healthcare operations.
• Experience supporting multi-site/regional operations is highly desirable.
• Strong customer service and communication skills.

Responsibilities

• Own regional clinical quality, monitor care delivery and implement practice changes.
• Track patient outcomes, readmission rates, infection rates, and satisfaction; act on trends.
• Lead performance improvement projects from problem identification to results.
• Standardize best practices across branches with repeatable processes.
• Coach and develop clinical teams with feedback and education.
• Prepare the region for surveys and accreditation reviews with ongoing readiness.
