Job summary

21st Century Home Health Services is seeking a Regional Quality Manager to lead quality across multiple California regions onsite at Walnut Creek, San Jose, Burlingame, or Sacramento. You will build and govern systems for better patient care, ensure readiness for surveys, and drive data-driven improvements across branches.

The role emphasizes coaching clinical teams, incident investigation, and aligning quality goals with staffing and growth plans to sustain improvements in a growing home health

Qualifications

• California Clinical License (RN, PT, ST, OT) is required.
• Bachelor's degree in healthcare administration, business, nursing, or related field is a plus.
• 2+ years in home health or healthcare operations is highly preferred.
• Multi-site or regional support experience is a plus.
• Outstanding customer service skills are required.

Responsibilities

• Own clinical quality across the region and monitor care delivery.
• Track patient outcomes, readmission rates, infection rates, and satisfaction to drive action.
• Lead regional quality initiatives from problem ID to measurement of results.
• Standardize best practices across branches and ensure repeatable processes.

Education

Bachelor's degree in healthcare administration, Business, Nursing, or related is a plus
