Qualifications

California clinical license preferred (Nursing, PT, OT, or ST).
2+ years in home health or healthcare operations.
Multi-site or regional support experience a plus.
Outstanding customer service skills.

Responsibilities

Own the regional quality function and set standards across sites.
Monitor care delivery via chart audits, observations, and outcomes.
Track patient outcomes, hospitalizations, readmissions, infection rates, and satisfaction.
Lead improvement projects from problem identification to results.
Standardize best practices with repeatable field workflows.
Coach and develop clinical teams with real-time feedback.
Investigate incidents with a systems lens and implement safeguards.
Partner with regional leadership to align quality with staffing and growth.
Prepare the region for surveys and accreditation through readiness.
Build and maintain dashboards, tools, and QAPI structure.
Foster accountability and psychological safety among staff.
