Job summary

21st Century Home Health Services Inc. seeks a Regional Quality Manager to lead clinical quality across the Northern California region, including Marin and Sonoma counties. You will build systems that support care delivery in the field, not just on paper, while driving performance improvement and accountability.

The role emphasizes coaching clinical teams, conducting audits, and preparing the region for surveys and accreditation. On-site work at local offices is required.

Qualifications

• California Clinical License (Nursing, PT, ST, OT) is highly preferred.
• Bachelor's degree in healthcare administration, business, nursing, or related is a plus.
• 2+ years in home health or healthcare operations is highly preferred.
• Multi-site or regional support experience is a plus.
• Outstanding Customer Service Skills.

Responsibilities

• Own clinical quality across the region and monitor care delivery.
• Track patient outcomes, hospitalization and readmission rates, infection rates, patient satisfaction, and star ratings.
• Participate in performance improvement projects from problem identification to measurement of results.
• Standardize best practices across branches and ensure repeatable processes in the field.
• Coach and develop clinical teams through real-time feedback and targeted education.
• Investigate incidents and adverse events with a systems lens and implement safeguards.
• Partner with regional leadership to align quality goals with staffing and growth plans.
• Prepare the region for surveys and accreditation reviews with ongoing readiness.
