Job summary

24 Hour Home Care is seeking a Regional Manager, Community Partnerships to lead day-to-day sales and leadership functions of the Community Partnerships team within the Disability Services Division. You will oversee CPMs, drive referral growth, and meet KPI and sales targets while partnering with leadership to optimize strategy and performance.

You will foster accountability and collaboration, lead field-based teams regionally, and build strong referrals through community outreach and stakeholder

Qualifications

• Bachelor’s degree required in Business, Healthcare, or related field.
• Minimum of 5 years of sales and/or management experience in healthcare or related industry.
• Experience leading, coaching, and developing sales or outreach teams.
• Strong understanding of sales strategy, KPI management, and pipeline forecasting.
• Experience in community-based services or healthcare environments preferred.
• Proficiency with Microsoft Office and Salesforce CRM preferred.

Responsibilities

• Lead, coach, and develop Community Partnerships Managers to drive referral growth and achieve Regional Center sales goals and KPIs.
• Monitor team performance through reporting, dashboards, pipeline reviews, and regular performance discussions to optimize productivity and accountability.
• Partner with VP to support sales strategy, forecasting, budgeting, and growth initiatives across the Disability Services Division.
• Support community outreach activities to strengthen referral partnerships and brand presence.
• Recruit, onboard, train, and retain high-performing team members while supporting professional development and performance management.
• Build cross-functional partnerships with Operations, Talent Acquisition, and other stakeholders to support service growth and alignment.
• Drive process improvement, CRM utilization, and strategic sales initiatives to improve efficiency and reporting.
