Job summary

24 Hour Home Care is seeking a Community Partnerships Manager to drive referral growth and admissions through strategic partnerships with Regional Centers and community partners. You will lead sales strategies, expand service lines, and collaborate with operations to align capacity with opportunities.

The role requires building relationships, presenting to partners, and managing pipelines while meeting regulatory requirements. Travel 5-7x monthly and hybrid work in Oxnard, CA are involved.

Qualifications

• Bachelor’s degree in Business, Marketing, Healthcare Administration, or related field.
• 2+ years of experience in sales, business development, referrals, or growth-focused role.
• Proven track record of meeting or exceeding goals.
• Spanish fluency preferred; experience with Regional Centers, healthcare, home care, or social services preferred.
• Experience using CRM systems, preferably Salesforce, with strong presentation and relationship-building skills.

Responsibilities

• Drive referral, admissions, and revenue growth by developing and executing strategic sales plans for assigned Regional Centers and community partners.
• Build and strengthen relationships through presentations, meetings, networking events, and ongoing outreach that increases brand awareness and referral activity.
• Identify and pursue new business opportunities, including service line expansion, community partnerships, and philanthropic initiatives that support organizational growth.
• Partner with Operations and leadership to align sales activity, pipeline management, and market opportunities with operational capacity and business objectives.
• Track, analyze, and report sales performance, referral trends, and pipeline activity while ensuring compliance with Regional Center, HIPAA, and regulatory requirements.
