Enhabit Home Health & Hospice seeks a Community Care Regional Program Director to lead regional outreach with senior living facilities and physicians. You will drive growth, implement community care programs, and monitor performance with clinical and operational teams to meet annual referral goals.

The role requires nursing or therapy licensure, regulatory knowledge, and strong relationship-building and communication skills. Home health/hospice experience is preferred.

Qualifications

• Must be a graduate of an approved school of nursing or therapy.
• Must be licensed in the state where they currently practice (therapists include PT/OT/SLP).
• Experience with federal/state/local laws governing home health/hospice operations.

Responsabilités

• Represent the region with senior living and health care facilities to apprise them of Medicare services.
• Collaborate with leadership to strategize growth and implement community care programs.
• Monitor program execution through quality assurance visits with referral sources and teams.
• Meet or exceed annual referral and admission goals set by senior management.
