PanCare of Florida is seeking a Referral Coordinator to bridge patients, providers, and partners. You will coordinate referrals, manage discharges, document authorizations, and uphold HIPAA standards while communicating with care teams.

Ideal candidates have at least 6 months in referral coordination and EHR/scheduling software; a high school diploma is required, with an Associate's degree preferred. This on-site role supports our Gulf Coast clinics and patient-centered care.

Qualifications

• High School Diploma or equivalent.
• At least 6 months of experience in referral coordination.
• At least 6 months experience with EHR systems and scheduling software.
• Knowledge of medical terminology, patient referral processes, and HIPAA compliance.

Responsabilités

• Coordinate and process referrals.
• Coordinate and process hospital discharges.
• Submit and track authorizations.
• Interface with corporate management, administrative personnel, medical, dental and behavioral staff.
• Identify health needs of the patient and family expectations.
• Assess resources and support networks for the patient.
• Assist in development and revision of treatment plans and monitor progress.
• Maintain an active caseload and procure additional services as needed.
• Review and facilitate mental health referrals from other sources where appropriate.
• Maintain patient and program records per standards and regulations.
• Maintain confidentiality and professional conduct.
• Participate in continuing education and stay informed in area(s) of expertise.
• Attend meetings and meet with supervisors regularly.
