Premise Health Inc. is hiring a Full Time Referral Coordinator for our Health Center in Orlando, FL.

You will collaborate with the health services team to identify and coordinate member care goals, secure access to medical and social services, schedule appointments, and arrange transportation as needed. You will document encounters in Epic, participate in case conferences, and maintain up-to-date plans to support member stability.

Qualificações

• High School diploma or GED is required.
• Experience accessing and navigating community resources preferred.
• Ability to work in and create a positive attitude environment, manage caseload, complete tasks/duties and address conflicts as they arise.

Responsabilidades

• Identify and prioritize member's health care goals with the care team.
• Facilitate access to medical and social services and coordinate appointments.
• Liaise with City agencies, residents, and community organizations.
• Document encounters and plans in Epic.
• Coordinate meetings with partners to move members toward stability.
• Assist scheduling appointments and arranging transportation.
• Participate in case conferences and provide status updates.
• Maintain up-to-date documentation and plan progress.
