Qualifications

• 6 months of referral coordination experience.
• 6 months experience with EHR systems and scheduling software.
• Knowledge of medical terminology, patient referral processes, and HIPAA regulations.

Responsibilities

• Coordinate and process referrals.
• Coordinate and process hospital discharges.
• Submit and track authorizations.
• Interface with management and clinical staff.
• Identify patient health needs and support networks.
• Assist in treatment plan development and monitoring.
