Job summary

Palms Medical Group is seeking a dedicated Case Manager to coordinate patient referrals, secure authorizations, and support continuity of care across services. This role emphasizes patient-centered service, HIPAA compliance, and collaboration with clinicians and external specialists.

The position requires a High School Diploma and experience in insurance verification/referrals; professional development opportunities and a supportive team environment are offered.

Qualifications

• General development: must be organized, a self-starter and detail oriented.
• Must work independently and as part of a team and multitask.
• Knowledge of referral sources in one’s geographical area.
• Ability to train and mentor co-workers by sharing knowledge.
• Experience with HIPAA and privacy regulations is preferred.

Responsibilities

• Provide professional, courteous patient support per PMG policies.
• Answer, screen, and route calls promptly (within 3 rings).
• Operate a multi-line phone system and document interactions in the EHR.
• Maintain knowledge of PMG departments and services.
• Follow referral policies, prioritize by urgency, and ensure complete data is submitted to specialists.
• Coordinate insurance authorizations and maintain medical necessity documentation.
• Identify community resources and assist patients in language or social barrier navigation.
• Track referrals in the EHR and close open orders per PMG policy.
• Perform administrative tasks and support audits as directed.
