Job summary

GEO Reentry Services LLC in California is seeking a Case Manager to provide direct casework services to STOP participants, including placement assessments, CMP development, and ongoing case management with supervision from the Clinical Team.

You will coordinate with outside agencies, maintain progress notes, conduct CMP reviews, and develop discharge and aftercare plans to support reintegration into the community, while upholding ethical standards and program requirements.

Qualifications

• High school diploma or equivalent and 3+ years working with criminal justice populations.
• Bachelor's degree in Human Services or related field preferred.
• Valid driver's license required.
• Alcohol and Drug Counselor Certification (DHCS) preferred.
• Understanding CBT and addiction dynamics for treatment planning.

Responsibilities

• Provides interactive services to STOP participants.
• Conducts placement assessments with participants.
• Develops Case Management Plans (CMP) with participants.
• Develops and monitors progress relative to CMP through all program phases.
• Makes appropriate referrals to outside agencies as necessary.
• Maintains progress notes in participant files.
• Keeps the AOR apprised of participant's progress.
• Develops discharge and aftercare plans.
• Documents participant case files and uploads required information.
• Collaborates with CBP program assignments and community reintegration.
