GEO Reentry Services LLC. is seeking a dedicated professional to facilitate placements for program participants transitioning from CDCR facilities to CBP programs, including LSUDD, LSUDT, FOTEP, sober living, and outpatient services.

The role emphasizes collaboration with parole officers, transportation coordination, and ensuring participants have the necessary medications and supplies prior to entry. Strong case management skills are essential.

Qualifications

• High school diploma or equivalent.
• Experience with case management and criminal justice populations preferred.
• Knowledge of CBT, addiction theories, and criminogenic risk factors.
• Able to read government regulations and write routine reports.
• Strong English spelling and communication skills.

Responsibilities

• Collaborates with Parole Officers to determine placement.
• Coordinates transportation to CBP program placement.
• Receives, tracks and monitors referrals from Parole Officers.
• Conducts assessment to appropriately place participants based on needs.
• Ensures participants have medication and supplies prior to entering facility.
• Completes required reports as required by CDCR and Program Manager.
• Performs other duties as assigned.
