Job summary

GEO Reentry Services LLC. is seeking a dedicated Case Manager to provide direct casework services for STOP participants, including placement assessments, CMP development, and ongoing progress monitoring within the CDCR framework.

The role requires collaboration with clinical staff, referral coordination, and documentation per GEO and CDCR standards to support successful reintegration into the community.

Qualifications

• High school diploma or its equivalent and a minimum of three (3) years of experience working with criminal justice populations.
• Bachelor's Degree in Human Services or a related field is preferred.
• Valid driver’s license required.
• Possession of an Alcohol and Drug Counselor Certification recognized by the DHCS is preferred.
• Understanding of CBT, addiction dynamics and criminogenic risk factors.
• Ability to tailor services to the participant's stage of change.

Responsibilities

• Provides interactive services to the STOP participants.
• Conducts placement assessments with participants.
• Develops Case Management Plans (CMP) with participants.
• Develops and monitors progress relative to CMP through all program phases.
• Makes appropriate referrals to outside agencies as necessary.
• Maintains progress notes in participant files.
• Keeps the AOR apprised of participant’s progress.
• Develops discharge and aftercare plans.
• Documents participant case files and uploads required information.
• Conducts monthly CMP reviews and updates with participants.
• Ensures implementation of program treatment/intervention under supervision.
• Manages and coordinates participant services through referrals and CBP program assignment.
• Collaborates with participants to identify a supporting network.

Education

Bachelor's Degree in Human Services or related field
