Qualifications

• One year Case Management experience preferred.
• Two years' general work experience providing support to individuals in behavioral health, Addiction Recovery or IDD services preferred.
• Valid driver's license required.

Responsibilities

• Act as part of a multidisciplinary team for service delivery.
• Assist in the development of recovery plans in collaboration with stakeholders.
• Provide community-based services focusing on education and resource connection.
• Complete all required assessments and documentation.
