Job summary

The Providence Center Case Manager BHOP Team Z2 will coordinate services for consumers to minimize stressors and to facilitate the reduction of psychiatric and substance abuse disorder symptoms. Provide support counseling, problem solving, contracts and limit setting.

Encourage consumers to attain the highest possible levels of independence. Responsibilities include coordinating care with a multi-disciplinary team, assisting with crisis interventions, and supporting consumers in achieving

Qualifications

• Associate's degree in Human Services or related field is required.
• Bachelor's degree is preferred.
• Experience in human services field is preferred.
• Bilingual Spanish is preferred.

Responsibilities

• Coordinate services for consumers to minimize stressors and reduce psychiatric or substance abuse symptoms.
• Provide support counseling, problem solving, contracts and limit setting.
• Assist consumers with goal setting, planning, and progress in recovery.
• Collaborate with agencies and a multidisciplinary team to develop and implement treatment plans.
• Take part in crisis assessment and risk management as needed.
• Assist with scheduling, transportation, and coordination of services.
