Job summary

The Providence Center Inc is seeking a Residential Case Manager on Atwood Street Per Diem to provide client-centered, recovery-oriented case management within a multidisciplinary team. Qualifications

• High School or GED required.
• 1–3 years of experience in a behavioral health setting; residential experience preferred.
• Valid driver's license, registration, current inspection and insured automobile.
• Strong knowledge of community resources and systems (housing, education, employment, healthcare).
• Ability to build rapport with diverse clients; cultural competence essential.
• Effective communication, written and verbal, across multidisciplinary teams.
• Excellent organizational, problem-solving, and time management skills.
• Proficiency in documentation and electronic record-keeping systems.
• Ability to travel within the community with reliable transportation.
• Flexible hours including evenings, weekends, holidays; 24/7 coverage when needed.

Responsibilities

• Coordinate and deliver person-centered, recovery-oriented case management as part of a multidisciplinary team.
• Work with the care team to align services with treatment plans.
• Conduct comprehensive assessments of housing, education, employment, finances, healthcare, and resources.
• Develop and implement individualized care plans with clients and families.
• Provide support counseling for mental health, substance use, and medical conditions.
• Monitor progress and follow-up; implement emergency protocols when required.
• Assist with daily activities and life skills; medication management and budgeting as appropriate.
• Supervise medication self-administration using the 6 Rights approach.
• Assist group home clients with medical needs and hygiene, conduct daily inspections.
• Facilitate socialization and recreational activities; connect clients to community programs.
• Schedule appointments with the Center and community partners; connect to social services.
• Advocate for client needs and maintain family contacts and relationships.
• Provide education to clients and families on navigating healthcare, education, and social services.
• Identify and respond to clinical emergencies; perform crisis intervention when needed.
• Maintain accurate records and prepare reports for audits and program evaluation.
• Attend team meetings; foster relationships with Day Program staff and partners.
• Transport clients to appointments; handle facility-related work requests as needed.
• Participate in trainings, case presentations, and agency initiatives.
