Job summary

The Providence Center seeks a Residential Case Manager to provide clinical case management in a residential setting, partnering with a multidisciplinary care team to support client independence and progress.

The role requires experience in behavioral health, strong communication, and the ability to navigate housing, employment, and healthcare resources within the Rhode Island community.

Qualifications

• High School diploma or GED required.
• 1–3 years of experience in a behavioral health setting; residential facilities preferred.
• Valid driver's license, vehicle registration, current inspection and insurance.
• Strong knowledge of housing, education, employment, healthcare resources.

Responsibilities

• Coordinate and deliver person-centered, recovery-oriented case management as part of a multidisciplinary team.
• Collaborate with care team to align services with treatment plans.
• Conduct comprehensive assessments to identify client needs in housing, education, employment, finances, healthcare and community resources.
• Develop, present and implement individualized care plans with clients, families and care team.
• Provide support counseling for mental health issues, substance use disorders, and medical conditions.
• Maintain accurate records and prepare reports for audits and program evaluation.
