Job summary

The Providence Center is seeking a Residential Case Manager to provide clinical case management in a residential setting, coordinating with a multidisciplinary care team to support recovery and independence. You will assess client needs across housing, education, employment, healthcare, and community resources, develop individualized care plans, and monitor progress with crisis intervention when needed.

High School or GED required.
1–3 years in a behavioral health setting; residential experience preferred.
Valid driver’s license, registration, current inspection, and insured vehicle.
Strong knowledge of community resources: housing, education, employment, healthcare.
Ability to build rapport and trust with diverse clients and partners.
Cultural competence and commitment to equitable, inclusive services; strong written and verbal communication.

Responsibilities

Coordinate person-centered, recovery-oriented case management within a multidisciplinary team.
Align services with clients’ mental health treatment plans.
Assess housing, education, employment, financial stability, healthcare, and community resources.
Develop and implement individualized care plans with clients, families, and care team.
