Job summary

The Providence Center is seeking a Residential Case Manager for the Women's Road to Recovery program in Rhode Island. This role provides clinical case management to support program goals through a client-centered approach, addressing mental health symptoms and co-occurring disorders.

You will work with a multidisciplinary team to develop individualized care plans, connect clients with housing, education, employment, and healthcare resources, and promote independence through education and life

Qualifications

• Experience in behavioral health settings, preferably residential facilities.
• Ability to communicate clearly and document accurately in EMR.
• Strong organizational and time-management abilities.

Responsibilities

• Coordinate person-centered, recovery-oriented case management as part of a multidisciplinary team.
• Align case management services with clients' mental health treatment plans.
• Conduct comprehensive assessments of housing, education, employment, and healthcare needs.
• Develop and implement individualized care plans with the care team and families as appropriate.
• Provide support and counseling for mental health, substance use, and medical concerns.
• Monitor progress and respond to crises or emergencies as needed.
• Assist with daily living activities and life skills to promote independence.
• Transport clients to appointments and community activities when required.
• Maintain records and prepare reports for audits and program evaluation.
• Collaborate with community partners to connect clients to social services and resources.
