Job summary

Resources for Human Development is seeking a Case Manager to assess participant needs, coordinate resources, and implement individualized service plans. You will advocate for participants and collaborate with families, providers, and multidisciplinary teams to promote recovery and well-being.

The role requires a bachelor’s degree in a related field, at least two years of case management experience, and strong documentation and communication skills. Travel to community settings will be frequent.

Qualifications

• Bachelor’s degree in a related field required.
• Minimum of two (2) years of relevant human services or case management experience.
• Intermediate computer proficiency with electronic documentation systems.
• Valid driver’s license and ability to pass criminal background checks.

Responsibilities

• Participate as an active member of a multidisciplinary team, providing services from a holistic, person-centered perspective.
• Conduct assessments to determine resources needed to meet participants’ individualized needs.
• Develop, implement, and review recovery plans with participants, supports, and stakeholders.
• Advocate for participants to ensure access to services and community resources.
• Maintain participant confidentiality and comply with HIPAA regulations and organizational policies.
• Deliver at least 75% of participant contacts within community settings.
• Provide crisis intervention services as needed.
