Job summary

Banyan Treatment Centers in Wasilla, AK is seeking an Aftercare Coordinator to guide clients after discharge, coordinating next steps and liaising with referral sources and families to ensure a solid continuity of care.

You will document interactions in the EMR, collaborate with clinical and outreach teams, and help secure resources, transportation, and appointments to support ongoing recovery. This is a full-time, daytime role with competitive hourly compensation and a nationwide network.

Qualifications

• Requires high school diploma or GED; Associate's degree preferred.
• Experience in behavioral health or customer service is a plus.
• Strong written and verbal communication skills are essential.
• Familiarity with Salesforce and/or Kipu EMR is a plus.

Responsibilities

• Coordinate Next Steps: Manage each client’s discharge plan and transition to the next level of care with clinical staff and the Outbound Team.
• Serve as a Key Liaison: Communicate with external agencies to ensure continuity of care.
• Maintain Accurate Documentation: Record interactions in the EMR to support post-discharge planning.
• Advocate for Retention & Safety: Support AMA blocking efforts and refer discharged clients to appropriate providers when needed.
• Engage Families & Referral Sources: Keep families informed and maintain updates with partners.
