Job summary

Banyan Treatment Centers in Wasilla is seeking an Aftercare Coordinator to support clients after discharge, coordinating next steps and ensuring a clear recovery plan. This full-time role runs Monday–Friday, 8:00am–4:30pm, and reports to the Executive Director, with compensation at $25-$28 per hour.

You will liaise with external agencies, document in the EMR, collaborate with clinical and outreach teams, and advocate for retention and safety while helping families navigate post-treatment

Qualifications

• High school diploma or GED; Associates degree preferred.
• Experience in behavioral health or customer service.
• Excellent written and verbal communication skills.
• Familiarity with Salesforce and/or Kipu EMR is a plus.
• Knowledge of local community resources.

Responsibilities

• Coordinate discharge planning and transition to next level of care.
• Serve as liaison with referral sources and families.
• Maintain accurate documentation in EMR (Kipu).
• Collaborate with clinical and operations teams; participate in weekly case consultations.
• Ensure confidentiality and regulatory compliance.
