Job summary

True North Recovery Inc. in Wasilla, Alaska, seeks a dedicated case manager to support participants in addiction and mental health programs. You will advocate, arrange resources, document care, and coordinate with partners to ensure comprehensive service delivery.

The role requires an associates degree in human services or related field, ability to obtain CDC II, and a valid Alaska driver’s license. Strong communication and coordination skills are essential.

Qualifications

• Requires an Associate degree in Human Services or related field (or equivalent experience).
• If in recovery, one year of sobriety and active participation in a recovery program is required.
• Must obtain CDC II within 3 years of hire.

Responsibilities

• Maintain relationships with partnering agencies and referral sources.
• Document all billable interactions in AKAIMS using DAP format.
• Coordinate with referrals, family, and community agencies to tailor treatment and case management.
• Provide motivational interviewing and emotional support to participants.
• Create crisis stabilization plans and coordinate peer supports as needed.
• Assist participants in linking to housing, employment, and other services.
• Develop personalized plans for program completion, focusing on housing, employment, and ongoing care.
• Assist in identifying grants for housing, transportation, and education.
• Attend trainings and staff meetings for continuous service improvement.
• Transport participants for activities and maintain professional conduct.
