Qualifications

• Experience in behavioral health or related setting preferred.
• Strong written and verbal communication skills with attention to detail.
• Familiarity with EMR systems and care coordination processes.

Responsibilities

• Coordinate discharge plans and next level of care with clinical staff and outbound team.
• Collect consents, IDs, and financial documentation to support continuity of care.
• Refer discharged patients to appropriate providers when needed.
