We’re seeking a dedicated and highly organized Aftercare Coordinator to support clients as they transition through every step of their treatment journey. In this key role, you’ll coordinate the next level of care, serve as a liaison with referral sources and families, and ensure that each client leaves with a clear, well-executed plan for continued recovery.
