Job summary

Banyan Treatment Centers is seeking a dedicated Aftercare Coordinator to guide patients as they move through each stage of recovery. The role emphasizes discharge planning, continuity of care, and collaboration with clinical teams, families, and external partners to ensure a clear, well-coordinated aftercare plan.

The coordinator supports long-term recovery by preventing gaps in care and addressing post-treatment needs with care, accuracy, and compassion, while coordinating with referrals, EMR

Qualifications

• High school diploma or GED required; Associate’s degree preferred.
• Experience in behavioral health, customer service, or familiarity with recovery-based programs.
• Strong written and verbal communication skills.
• Familiarity with Salesforce and/or Kipu EMR.
• Knowledge of local community resources and aftercare support services.

Responsibilities

• Coordinate discharge plans and next level of care with clinical staff.
• Obtain required consents, identification, and financial documentation for continuity of care.
• Refer discharged patients to appropriate providers when necessary.
• Serve as liaison with external agencies and partners (e.g., VA, probation, attorneys, HR, EAPs).
• Communicate with referral sources about arrivals, discharge planning, AMA risks, and hospitalizations.
• Engage families during the discharge process with timely updates.
