Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field.
• One to two years of experience in case management, counseling, or human services.
• Experience working with individuals with substance use disorders, co-occurring mental health conditions, or in recovery-oriented settings.

Responsibilities

• Provide comprehensive case management and support services to clients participating in substance use disorder treatment programs.
• Work collaboratively with treatment teams, families, and community partners to promote recovery, independence, and long-term stability.
• Advocate for clients navigating healthcare, legal, social service, and other systems.
• Help clients access needed medical, psychological, social, educational, and vocational services.
• Maintain confidential client records and document services.
