IRON Recovery and Wellness Center in Trenton, NJ is seeking a full-time Case Manager to support individuals in recovery. You’ll provide comprehensive case management, help clients access medical, social, and vocational services, and coordinate with treatment teams, families, and community partners.

The role emphasizes advocacy, transition planning, crisis support, documentation, HIPAA compliance, and helping clients build independent living skills and stable connections.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, or a related field is required or preferred.
• 1–2 years of case management, counseling, or human services experience preferred.
• Experience with substance use disorders or recovery-oriented settings is desirable.

Responsibilities

• Provide comprehensive case management to clients in treatment programs.
• Advocate for clients navigating healthcare, social services, and legal systems.
• Help clients access medical, psychological, social, educational, and vocational services.
• Maintain confidential client records and complete progress notes.
• Collaborate with treatment teams, families, and external providers.
• Support youth-focused services and family engagement as assigned.
• Facilitate recovery-oriented groups and crisis intervention as needed.
• Ensure HIPAA and agency policy compliance in all duties.
