Ascension Recovery Services in Morgantown, WV is seeking a compassionate Case Manager to coordinate services for clients with substance use and co-occurring conditions. You’ll partner with a multidisciplinary team to connect clients with housing, benefits, and community resources while advocating for long-term recovery.

This role requires strong communication, organization, and adherence to trauma-informed care principles within a recovery-focused environment.

Qualifications

• High school diploma or equivalent required.
• Experience providing case management or care coordination in behavioral health or related services.
• Knowledge of community resources and use of an EHR.

Responsibilities

• Provide comprehensive case management services for clients with behavioral health needs.
• Coordinate discharge planning and transitions of care to support continuity of services.
• Facilitate treatment team meetings and communicate client progress and barriers.
• Connect clients with housing, employment, transportation, and community resources.
• Maintain accurate documentation in the Electronic Health Record (EHR).
• Support trauma-informed, recovery-focused treatment environment.
