Job summary

Bridgeway Behavioral Health Services, Inc. in Elizabeth, NJ, is seeking a Wellness Specialist to provide on-site wellness and recovery services within a multidisciplinary team.

The role includes case management, crisis support, and linkage to community resources, with a schedule of Monday–Friday, 9:00am–5:00pm and one evening shift. The position requires engaging individuals, assisting with WRAP or PAD planning, delivering education, and coordinating with clinicians to support recovery.

Qualifications

• At least one year post-degree work experience providing case management services for people with mental health and co-occurring disorders within the community setting.
• CPRP preferred; veterans preference; new graduates will be considered.
• Valid driver’s license and vehicle required.

Responsibilities

• Engage individuals into BCIS services using motivational and strengths-based approaches.
• Link and refer individuals to needed community services to divert from emergency care.
• Coordinate and maintain referral and community service and resource lists.
• Provide case management activities per Treatment Plan and linkages for aftercare services.
• Develop Wellness Recovery Action Plans (WRAP) or Psychiatric Advanced Directives (PAD).
• Offer mental health education, illness management, and medication education on-site.
