Qualifications

• Minimum of 2 years of case management experience in a behavioral health or substance use setting.
• Knowledge of HIPAA, confidentiality standards, and ethical guidelines in client care.
• Proficiency with Electronic Health Records (EHR) systems.

Responsibilities

• Assess clients and develop individualized care plans with the clinical team.
• Coordinate referrals to housing, employment, medical care, mental health, and community resources.
• Advocate for clients and support their treatment and recovery journey.
• Document all case management activities in the EHR and prepare progress reports.
