Job summary

Qualifications

• Experience in residential addiction treatment settings preferred.
• Ability to coordinate with multiple social service agencies.
• Strong record-keeping and documentation skills.

Responsibilities

• Provide case management for residential addiction treatment clients, including coordination with social services and court/probation.
• Open and manage welfare and Medicaid cases, assist with Section 8 housing, and arrange supportive services.
• Conduct group sessions, maintain charts per DMHAS and CARF guidelines, and participate in MDT meetings.
