Care Plus NJ, Inc. is seeking a dedicated behavioral health specialist to assist clients through navigating social services and health systems, linking to care, and supporting wellness goals. You will apply evidence-based approaches to aid in recovery.

Responsibilities include case management, coordinating services, and collaboration with a multidisciplinary team; you will document appropriately and help clients access community supports.

Qualificações

• Bachelor's degree in a related field.
• Experience working within a health care or behavioral health setting.
• Experience working with adults with acute symptoms of mental illness and with chronic healthcare conditions.
• A valid driver's license and must be insurable by the agency's automobile insurance.

Responsabilidades

• Facilitate and coordinate psycho-educational services for consumers and their family members relating to wellness, health promotion, prevention, skill-building, and recovery topics.
• Offer on-site and outreach services (including transport of consumers).
• Fulfill case management services, including coordinating or directly facilitating linkages to services, service coordination, and treatment enrollment assistance.
• Ensure consumers have community supports in place that fulfill a positive corroborative role.
• Communicate observations and information about the consumer in the consumer's service record and to team members.
• Participate in supportive counseling, crisis intervention, quality assurance activities, and cross-functional action teams.
• Participate in the assessment and evaluation of consumers at intake/admissions, during ongoing recovery, service delivery, and discharge.
• Implement evidence-based interventions (such as CBT, MI, etc.) and assessments to help individuals achieve their treatment goals, link to community resources, and provide inter-agency coordination of treatment.
• Collaborate with team and treatment providers to establish comprehensive bio-psycho-social assessments and care plans.
• Contribute to the formulation of consumer-centered care plans in collaboration with team members, containing relevant, measurable goals and realistic time frames for achievement; perform progress reviews and evaluate the effectiveness of goals.
• Work with administrative staff to ensure that all services are documented based on billing and regulatory standards.
