IRON Recovery and Wellness Center in Trenton, NJ is seeking a Full-Time Case Manager for Monday7Friday, 11ampm with schedule flexibility. You will support clients through recovery, navigating healthcare, social services, and employment, collaborating with treatment teams and families to coordinate services.

This role emphasizes advocacy, relationship-building, and helping clients build stable, independent lives.

Qualifications

• Bachelors degree in Social Work, Psychology, Counseling, Human Services, or a related field
• One to two years of experience in case management, counseling, or human services
• Experience working with individuals with substance use disorders is a plus

Responsibilities

• Advocate for clients navigating healthcare, legal, social service, and other systems.
• Provide comprehensive case management to support recovery and independence.
• Assist clients with obtaining public benefits and other community resources.
• Collaborate with treatment teams, families, and community partners.
• Document services and track client outcomes per agency standards.
• Maintain HIPAA-compliant records and protect client confidentiality.
