You will assess needs, coordinate services, advocate for resources, and help participants achieve meaningful goals, acting as a liaison among participants, families, providers, and internal programs.

Qualifications

• Bachelor's degree in a human services field is required.
• Valid driver's license and reliable transportation.
• Ability to work in both office and community-based settings.
• Proficiency with Microsoft Office and electronic documentation systems.

Responsibilities

• Conduct comprehensive assessments to identify participant strengths, needs, barriers, and goals.
• Develop, implement, and monitor individualized service and treatment plans.
• Provide ongoing case management services to support participant progress and success.
• Coordinate services across internal programs, healthcare providers, community agencies, and support systems.
• Facilitate referrals and connect participants with appropriate treatment, recovery, and supportive services.
