Resources for Human Development (RHD) in Brick, NJ, is seeking a dedicated Case Manager to coordinate services and support individualized recovery goals for participants in behavioral health programs.

You will work with clinical staff, participants, and community providers to implement service plans, advocate for needs, and connect individuals with essential resources while maintaining thorough documentation and professional relationships.

Qualifications

• Bachelor’s degree in human services or related field.
• Minimum two years of experience providing support to individuals in behavioral health or related services.
• Intermediate computer skills and a valid driver’s license.
• Ability to travel and transport program participants.
• Ability to work in community-based settings and drive participants as needed.

Responsibilities

• Participate in multidisciplinary teams to provide holistic, values-based services.
• Develop and review individualized recovery plans with participants and staff.
• Advocate for access to services and support participant goals.
• Complete assessments and maintain documentation per standards.
• Transport participants to appointments as required.
