Job summary

HealingUS Communities is seeking a Pathways to Recovery Case Manager for a full-time, in-person role based in Farmingdale, NJ. You will be the main point of contact for participants from enrollment through training, employment and follow-up, providing encouragement and practical guidance.

Responsibilities include assessing goals, coordinating training and services, assisting with resumes and applications, and maintaining progress notes and follow-ups to support recovery and employment outcomes.

Qualifications

• Must have lived experience with addiction or mental health recovery, or a degree/certification in social work.
• Candidates should communicate clearly, build supportive relationships, and maintain professional boundaries.
• Strong organization and timely documentation are essential; comfort using email and digital records is required.

Responsibilities

• Meet with participants to identify goals and barriers; develop and update individualized employment plans.
• Coordinate career training and supportive services including transportation and supplies.
• Help participants prepare resumes, complete applications, and practice interviewing.
• Maintain regular contact to monitor progress and connect with recovery or community resources.
• Complete case notes and participant records; conduct 30-, 60-, and 90-day follow-ups.
