Job summary

Resources for Human Development is seeking a compassionate Case Manager to support individuals on their recovery and wellness journey. You will assess needs, coordinate services, advocate for resources, and help participants achieve meaningful goals, serving as a link between participants, families, providers, and internal programs.

This role requires strong communication, organization, documentation, and the ability to work in both office and community settings.

Qualifications

Bachelor's Degree in Human Services, Social Work, Psychology, Counseling, or a related field.
Valid driver's license and reliable transportation.
Ability to work effectively in both office and community-based settings.
Intermediate computer proficiency, including Microsoft Office applications and electronic documentation systems.

Responsibilities

Conduct comprehensive assessments to identify participant strengths, needs, barriers, and goals.
Develop, implement, and monitor individualized service and treatment plans.
Provide ongoing case management services to support participant progress and success.
Coordinate services across internal programs, healthcare providers, community agencies, and support systems.
Facilitate referrals and connect participants with appropriate treatment, recovery, and supportive services.
