Job summary

The Center for Prevention & Counseling, Inc. in Newton, NJ seeks a full-time Case Manager to support individuals with substance use disorders, coordinating access to treatment, housing, employment, and healthcare.

The role emphasizes person-centered services and effective collaboration with a Peer Recovery Specialist. Based in Newton with community outreach, the position requires strong communication, documentation, and the ability to travel within the community.

Qualifications

• Bachelor’s degree in health, psychology, social work, education, or other behavioral health professions.
• Minimum of three years of experience supporting individuals with substance use disorders or high-need populations.
• Knowledge of substance use disorders, recovery principles, and community-based resources.
• Ability to engage diverse individuals and work in a multidisciplinary team.

Responsibilities

• Conduct comprehensive case management assessments addressing housing, finances, transportation, legal services, employment, education, healthcare, and family needs.
• Provide weekly in-person and telephone outreach and support sessions.
• Use motivational strategies to encourage participation in services and recovery supports.
• Advocate to remove barriers and improve access to resources.
• Maintain timely, accurate documentation per agency and grant requirements.
• Participate in team meetings, supervision, and trainings.
