Qualifications

• Bachelor’s degree preferred; four years of related experience may substitute for degree.
• Minimum of two years of experience with behavioral health needs, substance use, homelessness, or related barriers.
• Valid driver’s license required.
• Strong verbal and written communication skills.
• Strong computer skills, including accurate data entry and documentation.

Responsibilities

• Build respectful, trusting relationships with clients and provide strengths-based case management support.
• Develop and follow individualized, person-centered plans focused on recovery, housing stability, wellness, and community connection.
• Help clients access treatment services, housing resources, mental health services, and other supports.
• Provide regular one-on-one case management sessions and monitor progress.
• Coordinate with clinical staff and community partners to support continuity of care.
• Maintain accurate documentation of assessments, service plans, and referrals.
• Participate in team meetings and continuous improvement activities.
