Job summary

My Hope for Tomorrow is seeking a Full Time Recovery Case Manager to coordinate aftercare planning, communicate with clients and families, and document progress in the EMR. The role involves weekly client meetings, scheduling calls, and ensuring smooth discharge and step-down processes.

Qualifications

• Bachelor’s Degree in Human Services, Criminal Justice or Psychology required.
• Extensive experience within Mental Health and Substance Use Disorder industry.
• Time management skills and the ability to work on multiple projects simultaneously.
• Coordinate with all departments and oversee clients stay at Hope for Tomorrow.
• Creation of an after-care plan that realistically meets the needs of the client.
• Vetting of client visitors and phone calls.
• Must meet then maintain all pre-employment requirements which include reference checks, applicable state and national required background screening, 10 panel urine drug screen, DMV check, insurance (if applicable), and license/credential verifications

Responsibilities

• Help hold clients accountable for their actions in the community and the completion of their work.
• Facilitate and provide timely documentation.
• Monitor compliance on client contracts, phone policy, “buddy” relations. Ensure that all pertinent information is notated on RSS office board at shift change.
• Ability to work in a fast-paced, dynamic environment.
• Ability to communicate effectively in written, electronic, and oral form.
• Ability to make sound decisions and/or find alternative solutions.
• Make weekly calls to the client’s family and log in the EMR; schedule calls/meetings.
• Meet with clients at least once a week to monitor the treatment process.
• Responsible for executing step downs and discharges.
• Uploading/delegating current ROIs and aftercare plans into EMR.
• Attend Morning Meeting, RCM Meeting, Supervision, Safety Squad and Quality Assurance meetings
• Client engagement beyond a single caseload; introductions within 48 hours of admission; interactions logged in EMR
