EVORAA HOLDINGS LLC is seeking a Case Manager to plan, implement, and coordinate individualized client services during residency and after discharge. The role includes psychoeducational and motivational instruction in groups and individual settings, with hands-on administrative support for the program.

The position is in-person, Monday–Friday, 7:30AM–4:00PM, requiring a Bachelor's degree and experience in case management, substance abuse; applicant must be detail oriented and capable of

Qualificações

• Knowledge of and experience with case management is required.
• Experience with substance abuse, mental health, housing, and community support services is required.
• Experience building rapport with substance-using populations and with referrals is strongly preferred.
• Experience managing patient services and records is strongly preferred.
• Compliance with internal, local, state, and federal requirements is strongly preferred.

Responsabilidades

• Perform patient screening, intake, orientation, and assessment.
• Develop or refine an individualized service plan and make referrals; arrange transportation and follow up for up to 30 days post-discharge.
• Provide individual case management sessions and document reviews in the client record.
• Assist patients in developing motivation for recovery and engaging in recovery activities.
• Coordinate with agencies, resources, and families to ensure continuity of care and follow-up.
