Qualifikationen

• Bachelor’s degree in social work or related field preferred; four years of experience may substitute.
• At least two years of experience with behavioral health, substance use, housing services or related fields.
• Valid driver’s license.
• Strong verbal and written communication skills.
• Ability to engage clients with empathy, cultural humility, and respect for voice and lived experience.
• Ability to collaborate effectively with clients, families, team members, and community partners.
• Knowledge of CARF CSTAR standards and quality improvement processes.

Aufgaben

• Build trusting, respectful relationships with clients through strengths-based engagement.
• Develop individualized service plans supporting recovery, housing stability, and community connection.
• Connect clients with treatment services, housing resources, mental health supports, and vocational resources.
• Provide regular one-on-one case management, monitor progress, and adjust plans with clients and the care team.
• Maintain accurate documentation including assessments, plans, progress notes, and referrals.
• Coordinate with clinical staff, housing providers, and community partners to ensure timely access to services.
• Promote client rights, confidentiality, and self-determination in all interactions.
