Job summary

Enhabit Home Health & Hospice is seeking a qualified licensed professional to review patient information during care transitions, provide education to LPs, and ensure regulatory documentation standards are met. You will train staff as needed and work to maintain strong relationships with referral sources and families.

In-office attendance is essential, with responsibilities spanning care coordination, documentation compliance, and staff development within a supportive team environment.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license per state requirements.
• Minimum of one year of experience.
• Medicare home health or hospice experience preferred.
• CPR certification required, valid driver license, and automobile liability insurance.
• Proficient with Microsoft Office and basic technology, including mobile devices.

Responsibilities

• Review and approve patient information submitted by the Licensed Professional (LP) during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to provide ongoing education and training to all branch LPs.
• Provide one-on-one training when necessary to ensure staff understand documentation requirements.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
