Job summary

Enhabit Home Health & Hospice is seeking a qualified professional to review and approve patient information submitted by Licensed Professionals during start of care, re-certification, resumption of care, and evaluation visits. The role partners with branch leadership to educate LPs on documentation requirements to meet regulatory standards.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Must maintain a valid clinical license per state requirements.
• Minimum of one year of experience required.
• Experience in Medicare home health or hospice setting preferred.
• CPR certification required.
• Valid state driver license and automobile liability insurance required.
• Basic technology skills including mobile device operation.
• Preference for experience with packet review/quality improvement and coding.
• Proficiency in Microsoft Office (Word and Excel).

Responsibilities

• Review and approve all patient information submitted by the LP during start of care, re-certification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate and train branch LPs on documentation requirements to meet regulatory standards.
• Provide one-on-one training when necessary.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
