Job summary

Enhabit Home Health & Hospice is seeking a clinical professional to review patient information at key care milestones and ensure regulatory compliance. You will train branch LPs on documentation, support ongoing education, and help maintain strong relationships with referral sources and patients.

Ideal candidates have nursing or PT credentials, an active license, CPR, and at least one year in Medicare home health or hospice, with solid tech skills and a valid driver’s license.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license per state requirements.
• CPR certification is required.
• Minimum of one year of experience in Medicare home health or hospice.
• Valid state driver license and automobile liability insurance.
• Strong technology skills and familiarity with mobile devices.

Responsibilities

• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visits.
• Partner with branch leadership to educate and train LPs on documentation and regulatory requirements.
• Provide one-on-one training to LPs when needed.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and staff.
