• Graduate of an approved nursing or physical therapy program.
• Valid clinical license in the state.
• Minimum of one year of experience.
• Medicare home health or hospice experience preferred.
• CPR certification and valid state driver license required.
• Basic technology skills, mobile devices; MS Word/Excel proficiency.

Responsibilities

• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visits.
• Partner with branch leadership to educate and train LPs on documentation and regulatory standards.
• Provide one-on-one training as needed and promote quality patient care.
• Maintain strong relationships with referral sources, patients, caregivers and staff.
